At the request of the EBMT-NG Board, the Research Sub-Committee of the EBMT-NG commissioned a survey of current related donor management and care at European transplantation centres.
Responding transplant centres (n=64) detailed a great variety across Europe. Most transplant centres gave information about processes and procedures that were based on local (22%) or national sources (22%). The majority of related donors received their information about the transplant process from the recipient’s transplant team (60%); moreover, they were consented, in the majority of cases, by a variety of combinations primarily made up of healthcare professionals from the transplant team (52% consented by a doctor from recipient’s transplant team). Follow-up care for donors was varied but most centres offered some form of post-donation care (94%); although there were centre specific differences, follow-up care was identified as being predominately outpatient based. Specific concerns of both recipients and donors were poorly reported.
With no international regulatory standard or robust evidence-based guidelines, related donor management and care is subject to a great deal of variation, and without mechanisms for the monitoring of standards these variations cannot guarantee quality or consistency of care. Researchers argue that because of the unique position of the related donor, specific guidelines are required to guarantee that their needs are adequately met by transplant centres, and that the volunteer status of donors is maintained along with confidentiality and the access to unbiased information.
