For the registration of haematopoietic stem cell transplant (HSCT)
MED-A:
MED-A FOR HSCT (28 Jul 10) *Modified* Minor reordering of items to allow Stem cell source and Ex vivo manipulation to be submitted independently for each donor
Includes the First Report; Appendix; Disease Classification Sheets and Follow Up.
MED-B:
MED-B forms include all MED-A items and are divided into subsets by disease and type of HSCT
Each disease specific form contains the following:

MED-A FOR Cell Therapy (non HSCT) (13 May 09) The new Cell Therapy Registry (CTR) aims to collect data on fetal or adult stem cells, or progenitor cells used for treatment other than hematopoietic stem cell transplantation or donor lymphocyte infusion, as well as data on the clinical characteristics and outcome of the patients.
